Job summary

HealthForce Healthcare is seeking a Physical Therapy Assistant for a LTAC travel assignment in Hampton, New Hampshire. The role supports a long-term care/assisted living setting with a 13-week contract starting 11/23/2026, offering 35 guaranteed hours per week and a shift from 8:00 a.m.

to 4:30 p.m. You will implement therapeutic exercises, monitor patient progress, and collaborate with the rehab team under a licensed PT, ensuring adherence to the plan of care and safety regulations.

Qualifications

• Active PT Assistant license in NH or eligible for licensure.
• LTAC/long-term care setting experience preferred.
• Strong communication and teamwork; good documentation habits.
• Availability for 8:00 a.m. - 4:30 p.m. shift (5 days/wk) and 13-week assignment with 35 hours.
• Travel-capable for temporary assignment in a different location.

Responsibilities

• Provide physical therapy interventions under supervision of a licensed PT, following the plan of care.
• Implement therapeutic exercises, ROM activities, gait training, mobility work, and transfer coaching.
• Support activities of daily living and functional mobility goals as part of the rehabilitation plan.
• Monitor patient responses to treatment, document progress, and communicate changes to the supervising PT.
• Collaborate with rehab team and nursing staff to ensure coordinated, patient-centered care.
• Maintain accurate, timely documentation and comply with safety, privacy, and clinical regulations.
