Job summary

Healthforce is seeking a Physical Therapy Assistant for a travel LTAC assignment in Hampton, NH. The 13-week contract starts 11/23/2026 with 35 guaranteed hours per week and shifts from 8:00 a.m. to 4:30 p.m.

This role involves patient-focused rehab in a long-term care setting, with supervision by a licensed PT and ongoing credentialing support. Responsibilities include therapeutic exercises, mobility training, and documentation; travel is required and there is potential for extension based on

Qualifications

• Active Physical Therapy Assistant license in NH or eligible to obtain licensure.
• LTAC/long-term care/assisted living setting experience preferred.
• Strong communication, teamwork, and documentation habits.

Responsibilities

• Provide physical therapy interventions under supervision of a licensed PT according to the plan of care.
• Implement therapeutic exercises, ROM activities, gait training, mobility work, and transfers coaching.
• Support activities of daily living and functional mobility goals as part of the rehab plan.
• Monitor patient responses, document progress, and communicate changes to supervising PT.
• Collaborate with rehab team and nursing staff for coordinated patient-centered care.
• Maintain accurate, timely documentation and comply with safety, privacy, and clinical regulations.
