Job summary

Healthforce is seeking a Physical Therapy Assistant for a 15-week LTAC travel assignment in Santa Fe, New Mexico.

The position offers a guaranteed 35 hours per week, a 5x8 shift pattern, and a start date of 09/28/2026. You will implement PT plans, assist with therapeutic exercises, and document patient progress, with on-site clinical duties in a LTAC/skilled nursing setting.

Qualifications

• Graduation from an accredited Physical Therapist Assistant program (Associate degree or equivalent).
• Active or eligible Physical Therapy Assistant license/certification as required for practice in New Mexico
• CPR/BLS certification or equivalent as applicable
• Ability to work in a Skilled Nursing Facility/LTAC setting with medically complex adult patients
• Inpatient or LTAC experience preferred

Responsibilities

• Implement and monitor physical therapy treatment plans for medically complex adult patients under supervision
• Facilitate therapeutic exercises, gait training, balance activities, transfers, and ROM programs per established plan
• Assess patient progress, document outcomes, and communicate findings to supervising PT and the care team
• Assist with mobility and activities of daily living as appropriate, while maintaining safety and infection control
• Prepare, clean, and maintain treatment spaces and equipment; ensure documentation complies with facility guidelines
