Job summary

Healthforce is seeking a Physical Therapy Assistant for a 13-week LTAC travel assignment in Bedford, NH. Start date 11/16/2026. 35 guaranteed hours per week with shifts 08:00–16:30, in a Skilled Nursing Facility setting providing long-term acute care services.

The role requires NH licensure eligibility and LTAC or skilled-nursing experience is preferred. You will deliver PT services under supervision, document progress, and collaborate with an interdisciplinary team to support patient-centered

Qualifications

• Active Physical Therapy Assistant license in NH or eligible to obtain licensure
• LTAC or skilled nursing setting experience preferred
• Available to start 11/16/2026 for a 13-week assignment
• Can work 08:00–16:30 with 35 hours/week
• Strong communication and documentation for interdisciplinary care

Responsibilities

• Provide physical therapy services under supervision, following the plan of care developed by the supervising PT.
• Implement and adjust treatments for LTAC patients, including gait training, ROM, strength, and ADLs.
• Monitor patient progress, document responses to treatment, and communicate updates to the PT and interdisciplinary team.
• Maintain accurate documentation in the patient record and ensure regulatory compliance.
• Educate patients and families on exercises, safety, and home exercise programs; promote patient-centered care.
• Participate in rounds and care coordination with nursing, therapy, and rehab teams; assist with equipment maintenance and infection control.

Requirements

• Active Physical Therapy Assistant license in New Hampshire or eligibility to obtain licensure per state requirements
• LTAC or skilled nursing setting experience preferred
• Availability to start on 11/16/2026 and commit to a 13-week travel assignment
• Ability to work the shift of 08:00–16:30 with guaranteed 35 hours per week
• Strong communication, documentation, and collaborative skills for interdisciplinary care
