Healthforce Healthcare LLC is seeking a Physical Therapy Assistant for a Lebanon, New Hampshire LTAC assignment. The 13-week travel position begins 12/01/2026, with 35 guaranteed hours and a 5x8 day shift (08:00–16:30). You will work under a licensed PT in a skilled nursing LTAC setting, with weekly pay and payroll support.

Required: NH PTA license (or eligibility) and PTA program graduation. Prior LTAC/SNF experience preferred; strong communication and teamwork are essential as you coordinate

Qualifications

• Active Physical Therapy Assistant license in New Hampshire or eligibility for licensure.
• Graduation from an accredited PTA program.
• Experience in LTAC, SNF, or inpatient rehabilitation setting preferred.
• Ability to work 08:00–16:30 and travel to Lebanon, NH.
• Strong communication skills and ability to collaborate within an interdisciplinary team.

Responsibilities

• Provide physical therapy services under the supervision of the licensed PT, implementing and adapting treatment plans for LTAC patients.
• Assist with patient evaluations, therapy sessions, and progression of therapeutic activities, including gait training, balance, strength, and range-of-motion exercises.
• Monitor patient responses to therapy, document progress, and communicate changes to the supervising PT and interdisciplinary team.
• Support safe patient handling, transfers, and mobility aids; ensure appropriate use of equipment and adherence to infection control protocols.
• Collaborate with nursing, rehabilitation, and discharge planning teams to coordinate care and optimize patient outcomes.
