Responsibilities

• Conduct comprehensive evaluations to determine physical therapy needs and create individualized treatment plans.
• Implement PT interventions using appropriate, evidence-based techniques; monitor progress and adjust plans as needed.
• Serve as a co-leader in interdisciplinary teams to coordinate patient care and discharge planning.
• Maintain accurate clinical documentation, including evaluations, progress notes, and discharge summaries within established deadlines.
• Supervise assigned therapy assistants and ensure safe, compliant delegation of tasks.
• Communicate patient status, progress, and discharge needs with patients, families, and the care team.

Requirements

• Doctor of Physical Therapy (DPT) degree.
• Active Physical Therapy licensure in California (or applicable state(s) of practice); eligibility as required.
• CPR certification (BLS or equivalent) required.
• Minimum two years of recent experience in the field.
• Valid driver’s license and eligibility to drive; age 21+.
• Vaccination requirements per organizational policy; exemptions may apply.
• Ability to work in a Short Term Acute Care Hospital setting.
