Responsibilities

• Conduct initial evaluations, create individualized treatment plans, and set measurable goals for residents
• Plan and implement physical therapy interventions focusing on mobility, strength, balance, gait, and functional independence
• Monitor progress, adjust treatment plans as needed, and document outcomes in resident records
• Collaborate with an interprofessional team to coordinate care, discharge planning, and transitions of care
• Educate residents and families on home exercise programs, safety, fall prevention, and activity modifications
• Ensure adherence to safety, infection control, and documentation standards within the facility setting

Requirements

• Active, unrestricted Physical Therapist license in California
• Doctor of Physical Therapy degree or equivalent credential
• Ability to work effectively in a skilled nursing facility environment
• Willingness to participate in a travel assignment and meet assignment requirements
• Strong communication and teamwork skills
• Eligibility to work in the United States
