Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs to restore function and minimize disability.
• Supervise Physical Therapy Assistants when appropriate; participate in initial admissions/evaluation visits.
• Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans.
• Complete and timely documentation, including OASIS scores, and notify the Clinical Team Manager within established timelines.
• Maintain thorough patient records and discharge planning, adjusting plans as progress is made.
• Collaborate with an interdisciplinary team to deliver patient-centered care aligned with home health and hospice goals.
