Job summary

HealthForce Healthcare is seeking a Physical Therapist for a temporary travel assignment in Unity, New Hampshire, within a skilled nursing facility. The contract runs 13 weeks starting 11/09/2026, with 40 hours per week and a daytime schedule from 08:00 to 16:30.

You will assess residents, develop plans of care, implement therapy interventions, and collaborate with an interdisciplinary team to improve mobility and quality of life.

Qualifications

• Active physical therapy license to practice in the United States.

Responsibilities

• Assess residents and develop individualized physical therapy plans of care.
• Implement therapeutic interventions including therapeutic exercise, gait training, balance, and functional mobility activities.
• Reassess progress regularly, adjust plans, and document outcomes.
• Collaborate with an interdisciplinary team to optimize residents’ mobility and quality of life.
• Educate residents and families on home exercise programs and safe mobility strategies.
• Ensure documentation accuracy and maintain compliance with facility policies and safety standards.
