Job summary

Healthforce in Unity, NH seeks a Physical Therapist for a travel SNF assignment starting 11/09/2026 for 13 weeks. Work 40 hours weekly, days 08:00–16:30 in a skilled nursing facility setting.

You will assess residents, implement therapy interventions, and collaborate with an interdisciplinary team to optimize mobility and quality of life. Documentation and educational duties are included.

Qualifications

• Active physical therapy license in the United States.
• Experience in skilled nursing facility or long-term care settings preferred.
• Ability to work 40 hours per week on daytime schedule (08:00–16:30).

Responsibilities

• Assess residents and develop individualized physical therapy plans of care.
• Implement therapeutic interventions including exercise, gait training, balance, and mobility activities.
• Reassess progress, adjust plans, and document outcomes.
• Collaborate with interdisciplinary team to optimize residents' mobility and quality of life.
• Educate residents and families on home exercise programs and safe mobility strategies.
• Ensure documentation accuracy and compliance with policies and safety standards.
