Ardor Health Solutions is seeking a Travel Physical Therapist for a 14-week SNF contract in Unity, New Hampshire. The role offers a 40-hour workweek, 8-hour day shifts, and a $2,000 quick-start bonus for qualified candidates.

Ideal candidates enjoy post-acute rehabilitation with adult and geriatric patients, and are licensed in NH with BLS. Start dates vary; accommodations may be provided in a rural New England setting.

Qualifications

Licensed Physical Therapist (NH) with an active license.
Strong skills in geriatric post-acute rehab and mobility training.
Ability to perform patient evaluations and develop treatment plans.

Responsibilities

Perform PT evaluations and reassessments.
Develop individualized treatment plans based on goals.
Provide therapeutic exercise, gait training, balance activities, and mobility interventions.
Assist patients to improve strength, endurance, and independence.
Educate patients and caregivers on home exercise programs.
Collaborate with interdisciplinary rehab team and document timely.
Complete accurate patient documentation.
