Qualifications

• Current, active license to practice physical therapy in New Hampshire (unrestricted).
• Eligibility to work in the United States.
• Ability to work in a skilled nursing facility setting and fulfill a 40-hour weekly schedule.
• Strong written and verbal communication skills; proficient in relevant documentation and electronic records.
• Ability to collaborate with an interdisciplinary care team and adapt to a travel assignment environment.

Responsibilities

• Conduct comprehensive PT evaluations for residents and establish individualized treatment plans to address mobility, gait, strength, balance, and functional independence.
• Implement evidence-based therapeutic interventions, document progress, and adjust plans based on patient response and goals.
• Collaborate with an interdisciplinary team (nursing, OT, SLP, social work) to optimize care plans, discharge planning, and family education.
• Administer therapeutic modalities as appropriate, monitor safety, and ensure infection control and patient protection during therapy sessions.
• Educate residents and families on home exercise programs, safety strategies, and fall-prevention techniques; provide clear patient- and caregiver-facing instructions.
• Maintain accurate documentation in the medical record, meet regulatory requirements, and contribute to quality improvement and compliant care delivery.
