Job summary

HealthForce Healthcare is seeking a Physical Therapist for a 13-week travel assignment in Peterborough, New Hampshire. Start date November 23, 2026, with a 40-hour workweek and a daily shift from 08:00 to 16:30 in a skilled nursing facility.

This role focuses on comprehensive evaluations, evidence-based interventions, and collaboration with an interdisciplinary team to support mobility, gait, strength and safe discharge planning.

Qualifications

• Active physical therapy license in NH (unrestricted).
• Eligible to work in the United States.
• Experience in skilled nursing facility setting preferred.
• Ability to maintain 40-hour weekly schedule.

Responsibilities

• Perform comprehensive PT evaluations and develop individualized treatment plans.
• Deliver evidence-based interventions, document progress, adjust plans.
• Collaborate with nursing, OT, SLP, social work to optimize care and discharge.
• Administer modalities, monitor safety and infection control.
• Educate residents and families on home exercise programs and fall prevention.
• Maintain medical records and ensure regulatory compliance.
