Fusion Medical Staffing is seeking a Travel Physical Therapist for a 13-week assignment in Long Grove, IL. You will evaluate and treat patients, develop individualized plans, and collaborate with a multidisciplinary team to ensure high-quality care.

Qualifications

• Valid Physical Therapy license in compliance with state regulations.
• Current BLS certification (AHA/ARC).

Responsibilities

• Evaluate patients’ physical conditions using tests, history and exams to identify needs.
• Develop personalized rehabilitation programs based on assessments.
• Implement treatment plans to improve mobility and reduce pain.
• Document evaluations, interventions and outcomes in the medical record.
• Collaborate with physicians, nurses and therapists for coordinated care.
• Educate patients on home exercise programs and self-management.
