Job summary

Evolve Home Health in Agoura Hills, CA is seeking an Occupational Therapist to visit patients at home, with daily visits and adherence to standards of care.

Candidates must have ACOTE-accredited OT program, CA license, NBCOT eligibility, CPR, and willingness to travel extensively within the service area. The pay is $100.00 - $150.00 per hour per visit, and duties include documentation and coordination with the care team.

Qualifications

• Graduate of ACOTE-accredited OT program and NBCOT eligible or certified.
• Licensed as an OT in California.
• Minimum one year of clinical OT experience.
• Current CPR certification.
• Willing to travel up to 100% for business

Responsibilities

• Evaluate patient’s functional status and home environment; identify equipment needs and plan care.
• Develop and implement OT treatment programs with clear goals.
• Teach and retrain daily living activities and adapt activities as needed.
• Fabricate splints and instruct on adaptive equipment use.
• Train patients in prosthetic/orthotic device use where applicable.
• Maintain clinical records and communicate changes to physicians and care teams.
• Coordinate with other health professionals and participate in discharge planning.
• Supervise OT assistants and oversee home health aide care every two weeks if therapy is sole skilled service.
