Job summary

Evolve Home Health in Agoura Hills, CA, is seeking a Physical Therapist with home health experience to join our field team.

You will provide PT services per physician plans of care, assess functional status, train patients and caregivers, and participate in discharge planning.

Must hold California PT license, have CPR, and be willing to travel up to 100% of the time; pay rate $100–$120 per hour.

Qualifications

• Requires degree in physical therapy from an accredited program.
• Licensed to practice as a physical therapist in California.
• Minimum of one year of physical therapist experience; home health preferred.

Responsibilities

• Provide physical therapy services per physician's plan of care.
• Assess functional status and home environment.
• Evaluate muscle strength, mobility, gait, ROM.
• Instruct patients and caregivers in use of therapeutic devices.
• Discharge planning assistance and progress summaries.
• Supervise PT assistants and train home health aides every two weeks.
