Job summary

Caringhand Home Health Solutions, Inc is seeking an Occupational Therapist to help patients regain independence and safely navigate daily life in their homes. The role focuses on personalized treatment plans, patient education, and coordinating with the care team to deliver patient-centered home health care.

Per diem position with a rate of $130 per visit. Requires CA OT license, NBCOT registration, and ability to travel to patient homes.

Qualifications

• Graduate of an OT curriculum accredited jointly by AMA-Committee on Allied Health Education and Accreditation and the American Occupational Therapy Association.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
