Job summary

Caringhand Home Health Solutions, Inc. is seeking an Occupational Therapist to provide in-home therapy, helping patients regain independence and improve daily living skills in the comfort of their own homes.

Qualifications

• OT degree from ACOTE-accredited program.
• Active California OT license required.
• Current CPR certification (AHA).
• Must be willing to travel to patients' homes.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in activities of daily living affected by illness or injury
• Recommend adaptive equipment and prosthetic/orthotic devices
• Maintain accurate clinical records and report changes promptly
• Coordinate with the care team to ensure patient-centered care
• Educate family members and caregivers on the treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is sole skilled service
• Support discharge planning as patients transition from home health care
• Perform other duties as required
