Qualifications

• Minimum 1 year of recent Home Health experience required.
• Current OT license required (if applicable) and CA license in hand.
• Current BLS certification required.
• Must meet weekly productivity point requirements.

Responsibilities

• Provide OT services in Home Health for geriatric patients.
• Caseload typically 5–7 patients per day.
• Day shift: 5x8 hours, 8:00 AM to 4:30 PM; no weekends or on-call.
• Territory covers up to 60 miles from hub office; travel to assigned areas.
• Timely communication and documentation using HCHB EMR and Oasis.
