Job summary

Caringhand Home Health Solutions, Inc. is seeking a field-based Occupational Therapist to travel to patients' homes, evaluate functional abilities, and develop personalized treatment plans.

The role focuses on helping patients regain independence and safely navigate daily life in their own homes. The position requires California OT licensure, a minimum of two years of clinical OT experience, CPR (AHA), and a valid driver’s license.

Qualifications

• Licensed occupational therapist with California license
• Minimum 2 years of clinical OT experience
• CPR certification (AHA)
• Valid driver's license and reliable insured vehicle
• Strong communication and organizational skills

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in activities of daily living
• Recommend and instruct patients on adaptive equipment and orthotic devices
• Maintain accurate clinical records and report changes promptly
• Coordinate with the care team for patient-centered care
• Educate family and caregivers on the treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services
