Job summary

Healthforce Healthcare LLC is seeking an Occupational Therapist, General, for a travel assignment in Albuquerque, New Mexico. This 13‑week temporary role starts 12/01/2026 in a skilled nursing facility.

Weekly pay ranges from $2,250 to $2,385 with 35 guaranteed hours. Shift is 5x8, 08:00–16:30. Active NM OT licensure, SNF experience, and strong assessment and documentation skills are required; credentialing/background checks will be completed.

Qualifications

• Active OT license in New Mexico.
• Experience in skilled nursing facility settings.
• Strong assessment, documentation, and communication skills.
• Ability to work in an interdisciplinary team and fast-paced environment.
• Willingness to complete credentialing and background checks.

Responsibilities

• Perform comprehensive OT evaluations for residents in a SNF.
• Develop and monitor individualized treatment plans to improve ADLs, mobility, cognition, independence.
• Provide direct therapy and supervise rehab activities; adapt plans to progress notes.
• Document progress and outcomes in the medical record; ensure accuracy and compliance.
• Collaborate with PTs, SLPs, nursing, and physicians to coordinate care and discharge planning.
