Qualifications

• Active Occupational Therapist license in New Mexico or eligible for licensure.
• Ability to work in a skilled nursing facility environment.
• Strong communication and documentation skills; collaboration with an interdisciplinary team.
• Experience in geriatric or SNF settings preferred but not required.
• Willingness to travel for a travel assignment and adapt to varied resident needs.

Responsibilities

• Perform occupational therapy evaluations for residents in a skilled nursing facility and develop individualized treatment plans.
• Implement therapy focusing on activities of daily living, mobility, coordination, and perceptual skills; monitor progress and adjust plans as needed.
• Document assessments, goals, progress notes, and discharge planning in the electronic medical record.
• Collaborate with nursing staff, therapy assistants, and social work to coordinate comprehensive resident care.
• Educate residents and families on home exercise programs, safety strategies, and caregiver instruction.
• Support discharge planning and transition processes to promote continuity of care and appropriate follow-up.
