Job summary

Healthforce Healthcare LLC seeks a Traveling Occupational Therapist for a 13-week assignment in Albuquerque, NM. Start date 12/01/2026, 35 guaranteed hours per week, 5x8 shifts from 8:00 AM to 4:30 PM in a Skilled Nursing Facility.

License required in NM or eligible for licensure; expected travel to site and collaboration with an interdisciplinary team. Healthforce provides recruitment and assignment support for the duration of the contract.

Qualifications

• Active Occupational Therapist license in New Mexico or eligible for licensure.
• Ability to work in a skilled nursing facility environment.
• Strong communication and documentation skills; collaboration with an interdisciplinary team.
• Experience in geriatric or SNF settings preferred but not required.
• Willingness to travel for a travel assignment and adapt to varied resident needs.

Responsibilities

• Perform occupational therapy evaluations for residents in a skilled nursing facility and develop individualized treatment plans.
• Implement therapy focusing on activities of daily living, mobility, coordination, and perceptual skills; monitor progress and adjust plans as needed.
• Document assessments, goals, progress notes, and discharge planning in the electronic medical record.
• Collaborate with nursing staff, therapy assistants, and social work to coordinate comprehensive resident care.
• Educate residents and families on home exercise programs, safety strategies, and caregiver instruction.
• Support discharge planning and transition processes to promote continuity of care and appropriate follow-up.
