Job summary

Signify Health is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations across a defined coverage area, predominantly in patients' homes. Travel up to 100% across licensed states is expected, with overnight stays possible.

The role emphasizes patient engagement, health history review, physical evaluation, and coordination to connect members with the right care. Board-certified NPs/PAs with multi-state licensing are preferred.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses in coverage area, as required.
• Able to cover all counties within state and flex to other areas of business need.

Responsibilities

• Join a national network of purpose-driven clinicians dedicated to improving health outcomes.
• Visit millions of people nationwide to provide connections to the right care for unique needs.
• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products.
