Qualifications

• Active, unrestricted licenses in coverage area(s).
• Board certification required.

Responsibilities

• Conduct in-home health evaluations for patients.
• Travel up to 100% of the time to defined coverage areas.
• Use iPad and clinical workflow; follow prescribed protocols.
• Collaborate with Signify Health clinicians to coordinate care.
