Job summary

Signify Health is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations across a defined coverage area, with up to 100% travel. Clinicians will assess medical history, perform physical evaluations, and coordinate with health plans for additional tests as needed.

The role requires active NP or PA licenses, board certification, and multi-state licensing as applicable.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses in coverage area, as required.
• Able to cover all counties within state and flex to other areas of business need.

Responsibilities

• Build trusted relationships to improve health outcomes through in-home visits.
• Travel across a defined coverage area to assess and engage members.
• Work with Signify Health clinicians and support teams as needed.
• Conduct virtual visits when required by care needs.
• Provide Signify Health services including diagnostic and preventive evaluations.
