Job summary

Signify Health is recruiting a Full Time Clinician (NP/PA) to perform in-home health evaluations across a defined coverage area. You will visit patients in their homes, review medical history, conduct physical evaluations, and perform specified diagnostic tests as ordered by health plans.

The role requires active NP/PA licensure, board certification, and the ability to obtain multi-state licenses. Up to 100% travel is possible, including overnight stays where needed.

Qualifications

• Active, unrestricted licenses in coverage areas.
• Board certification (required).
• Ability to obtain and maintain multi-state licenses as required.
• Ability to cover all counties within state and flex to other areas of business need.

Responsibilities

• Build strong relationships with health plan members in home visits.
• Travel locally and across licensed states to conduct evaluations.
• Collaborate with Signify Health colleagues to deliver compliant care.
• Perform in-home health evaluations using iPad with clinical workflow.
• Provide diagnostic and preventive services as indicated by the health plan.
• Support the program by visiting a broad patient base to bridge gaps in care.
