Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP/PA) to perform in-home health evaluations across defined coverage areas. Travel up to 100% and lodging may be required, with multi-state licensure as dictated by business needs.

You will evaluate medication history, perform physicals, and engage health plans for additional services. Responsibilities include flexible travel, occasional virtual visits, and delivering comprehensive Signify Health services in alignment with plan

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses in coverage area, as required.
• Able to cover all counties within state and flex to other areas of business need.

Responsibilities

• Provide In-Home Health Evaluations in members' homes.
• Travel locally and nationally to defined coverage areas (up to 100%).
• Conduct medication and medical history review, physical evaluation, and select diagnostic tests as ordered.
• Utilize iPad and clinical workflow; virtual visits may be conducted as needed.
• Coordinate care connections based on health plan requirements.
