Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to provide in-home health evaluations across a defined coverage area. The role is travel-intensive, up to 100%, with overnight stays possible and multi-state licensing as required.

You will perform clinical evaluations using an iPad, review medical histories, and coordinate with health plans. Qualified candidates hold board certification, active licenses in the coverage area, and can cover counties within the state while

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Able to obtain and maintain multi-state licenses as needed.
• Able to cover all counties within state and flex to other business needs.

Responsibilities

• Bring your heart into every visit, joining a national network of clinicians dedicated to improving health outcomes.
• Visit members at home to connect one-on-one, review meds and medical history, perform physical evaluation.
• Conduct virtual visits as needed and provide diagnostic and preventive services.
• Travel locally and across licensed states up to 100% of the time; use iPad for clinical workflow.
