Job summary

Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. You will travel across a defined coverage area, perform 45-minute visits, and use an iPad-based workflow to document health data.

Must hold board certification and be licensed, with multi-state licensure as needed. Ideal candidates are flexible for local travel, perform physical evaluations, and may conduct virtual visits when appropriate.

Qualifications

• Must hold an active, unrestricted license in one or more coverage areas.
• Board certification is required.
• Ability to obtain and maintain multi-state licenses as needed.
• Willingness to travel up to 100% and cover counties within state.

Responsibilities

• Perform in-home health evaluations (~45 minutes) and review medications and history.
• Conduct physical examinations and, if ordered, related diagnostic tests.
• Travel locally and within licensed states; conduct virtual visits when needed.
• Maintain accurate documentation using Signify Health workflow.
