American Traveler, LLC. is hiring an LPN/LVN for a skilled nursing LTC unit in Fond du Lac, Wisconsin. The role requires 1+ year of nursing experience and BLS certification, with a focus on patient admission, care planning, and coordination with RN/MD.

Evening shift 14:00-23:00, every other weekend and holidays. You will perform tasks such as IV therapy, wound care, medication reconciliation, isolation precautions, and patient health history intake while supporting patients with degenerative

Qualifications

Responsibilities

• Admission and health history intake; communicate status changes to RN/MD
• Maintain isolation precautions and PPE
• Medication reconciliation and pain assessment/management
• Assess patients’ health and manage care for degenerative disorders (Alzheimer's, ALS, Parkinson's)
• Perform mouth care and pressure ulcer assessment/prevention
• Caring for patients with comorbidities and acute medical needs
• Use of PointClickCare charting software preferred
• Care for patients with end-of-life needs

American Traveler is hiring an LPN/LVN for a skilled nursing LTC unit requiring 1+ year of experience and BLS certification.

Requirements

• License required if applicable
• BLS certification required
• Minimum 1 year of relevant nursing experience
• Skills include foley catheter insertion, insulin administration, IV therapy/medication, neurological status assessment, dressing changes, skin/wound assessment, wound vac care, JP drain care, colostomy care, end of life/palliative care, CPAP/BiPAP management, Hoyer lift use, and diabetes monitoring
• Experience with starting and maintaining IVs preferred
• Experience with oxygen administration and tracheostomy care preferred
• Experience with feeding tube care and management preferred
• Experience with traumatic brain injury care preferred

Responsibilities

• Skilled nursing/long-term care facility unit
• Patients present with multiple comorbidities and acute medical care needs
• Uses PointClickCare charting software (preferred experience)
• Evening shift, 5x8 hours, 14:00-23:00
• Every other weekend and holidays required
• Responsibilities include patient admission, communicating status changes to RN/MD, isolation precautions/PPE, medication reconciliation, pain assessment and management, patient health history intake, and restraint use
• Care for patients with degenerative disorders such as Alzheimer's, ALS, and Parkinson's
• Perform mouth care and pressure ulcer assessment/prevention
• Any solid color scrubs permitted
• Candidates must live more than 70 miles from the facility to be eligible
• Clinicians cannot work past 1 year as a traveler and must take a 6-month break before returning
• First-time travelers will be considered
