You will deliver skilled nursing care during home visits, including wound care, vitals checks, medication administration, and documentation of plans of care under RN supervision. PA license or eligibility is required, with 2 years home care experience.

Minimum 2 years of home care experience
Proficiency in wound care
Ability to work in a home healthcare setting and cover 19104 and 19139 zip codes
Comply with agency policies, documentation standards, and patient safety guidelines

Responsibilities

Deliver skilled LPN nursing care during home visits, with a focus on wound care management and documentation.
Assess patient condition, monitor vital signs, administer prescribed medications, and implement plan of care under supervision.
Perform wound care procedures, dressing changes, and routine clinical tasks in accordance with clinical policies.
Maintain accurate and timely charting in the electronic medical record; communicate updates to the RN supervisor and care team.
Educate patients and families on care plans, medication administration, safety, and self-management strategies.
Coordinate with physicians, therapists, and other providers to ensure continuity of care and adherence to care plans
