Job summary

Yale NewHaven Health is seeking a Patient Care Associate to support direct and indirect patient care under supervision. You will gather patient information, assist in assessments, and help deliver compassionate, patient-centered care across the unit.

The role requires effective communication with patients and families, teamwork with the care team, and adherence to PFCC principles to ensure a positive care experience.

Qualifications

• Demonstrated ability to communicate effectively with patients, families, peers and supervisors.
• Must be able to prioritize and adjust work plans based on patient and unit needs.
• CPR certification required.

Responsibilities

• Assist clinical staff by obtaining and reporting patient information to aid assessment and planning.
• Perform designated technical duties to support safe and efficient patient care.
• Coordinate with multidisciplinary team to address patient/family needs and ensure PFCC.
• Serve as a front-line resource at the BA desk when required by supervisor.
