Job summary

Yale New Haven Health is seeking a Patient Care Associate to support nursing staff in direct patient care and the delivery of high-quality services. You will collect information, monitor patients, and assist with essential treatments under supervision.

The role requires working closely with physicians, patients, and families to ensure compassionate, patient-centered care and adherence to center policies. CPR certification is required.

Qualifications

• High school diploma or GED; CNA or equivalent required.
• Two years direct patient care experience or equivalent education.
• CPR certification required.

Responsibilities

• Under the Clinical Nurse, obtain and report information to assist assessment, planning and evaluation of patient needs.
• Obtain vital signs and, when delegated by the RN, monitor patient status.
• Assist in safe and efficient delivery of patient care and selected treatments.
• Collaborate with the multidisciplinary team to address patient and family needs.
• Cover the Information Associate desk in the absence of IA as directed.
• Attend mandatory continuing education classes.
