Job summary

Yale New Haven Health seeks a Patient Care Associate to support clinical staff and assist with essential patient care activities. Under the supervision of the Clinical Nurse, you will collect vital signs, assist with treatments, and help ensure safe, compassionate care.

You will collaborate with the multidisciplinary team to address patient and family needs and maintain strong communication with patients and colleagues.

Qualifications

• High school diploma or GED; CNA or equivalent (e.g. Medical Assistant, Phlebotomy Technician, student nurse with clinical experience).
• Additional experience preferred depending on placement.

Responsibilities

• Under the direction of the Clinical Nurse, perform duties to obtain, report information and assist in patient assessment, planning and evaluation.
• Obtain and record vital signs as delegated by the RN.
• Perform technical duties and selected treatments that contribute to safe patient care.
• Assist in care of patients with attention to safety and collaboration with the care team.
• Communicate effectively with patients, families and staff; cover BA desk in the absence of BA.
• Collaborate with multidisciplinary team to address patient/family needs respecting diversity.
