Job summary

Yale New Haven Health is seeking a Patient Care Associate to support patient care under the supervision of the Clinical Nurse and the Patient Services Manager. You will contribute to patient- and family-centered care (PFCC) and provide essential nursing support, including vital signs, ECGs, and coordination with the care team.

Qualified candidates have a high school diploma or GED with CNA or equivalent, 2+ years of direct patient care or equivalent education, and completion of required

Qualifications

• High school diploma or GED; CNA or equivalent (e.g., Medical Assistant, Phlebotomy Technician, student nurse with clinical experience)
• Two years of direct patient care experience or equivalent education
• Successful completion of Patient Care Associate Competency Based Training and Practicum and unit-specific competencies, including age-specific competencies

Responsibilities

• Under supervision of Clinical Nurse, assists in assessment, planning and evaluation of patient needs and response to treatment.
• Obtains and records vital signs as delegated by the RN.
• Obtains a 12-lead ECG and continuous Lead II with patient's information entered.
• Collaborates with multidisciplinary team to address patient/family needs and maintain PFCC principles.
• Covers the BA desk in the absence of the BA as needed.
• Communicates effectively with patients, families, and staff.
