Job summary

Intermountain Health is seeking a Clinical Program Coordinator in Colorado to assess, plan, implement, and coordinate treatment processes from diagnosis through follow-up. You will guide patients through collaboration with medical teams and provide educational support across the hospital stay and after discharge.

The role requires RN licensure, BLS-HCP, and at least three years of RN experience, with preferred trauma performance improvement background.

Qualifications

Bachelor's Degree in Nursing.
Current license as a Registered Nurse in the state of practice.
Current American Heart Association Basic Life Support for Healthcare Providers (BLS-HCP) certification on hire or within 60 days.
Three years of relevant RN experience.

Responsibilities

Coordinate assigned program; discuss with physicians and nurses; ensure care meets evidence-based guidelines.
Provide education to patients and families during hospitalization and after discharge as a resource.
Participate in patient care rounds.
Collaborate to oversee regulatory requirements; perform chart reviews and data analysis for performance improvement.
Develop and implement evidence-based policies and standards for the program.
Maintain and analyze data; act as subject matter expert; support a Quality Assurance program.
