Responsibilities

• Coordinate trauma patient care across hospital-to-discharge continuum.
• Assess, plan, implement, and evaluate nursing care for trauma patients.
• Document in the EHR and communicate with patients and families.
• Provide resources for physical and mental health healing.
• Conduct in‑person visits and phone calls.
