Qualifications

• A minimum of two years experience in an acute care setting.
• Casework in medical social work is preferred.
• Current State of Wisconsin license as a social worker is required.

Responsibilities

• Coordinate the discharge plan of care for all patients during their hospital admission.
• Provide biopsychosocial assessments and updates for discharge care needs.
• Establish a care plan in collaboration with the multidisciplinary team.
