Froedtert Health is seeking a Trauma Care Coordinator RN to provide comprehensive care coordination for trauma patients from admission through discharge and outpatient follow-up. Location: Milwauke, Wisconsin at Froedtert Hospital facility.
Employment: Full Time (1.0 FTE), 40.00 standard hours per week.
Shift: Shift 1 – Mon‑Fri 8:00 AM – 4:30 PM.
Provides comprehensive care coordination for trauma patients, supporting a holistic approach throughout the continuum of care. Responsibilities include assessment, planning, implementation, and evaluation of nursing care for trauma patients, supporting them from hospital through discharge and outpatient settings. Works with staff to provide resources for physical and mental health healing, documents patient and family support in the electronic health record, and conducts in‑person visits and phone calls. Other duties as assigned.
Minimum of 3 years of clinical nursing practice experience in a related setting/specialty required; 5 years preferred.
Bachelor's degree in nursing is required; Master’s degree in nursing or related field preferred.
Nursing assessment skills, creative, solution‑based problem solving, critical thinking, analytical judgment to identify unusual circumstances, strong customer service and interpersonal skills, ability to work independently and prioritize responsibilities, knowledge of local resources and system navigation, Microsoft software applications, effective written and verbal communication, managed care knowledge, and computer skills including EHR navigation, data management, presentations, Excel, and report writing preferred.
Requires a current Wisconsin Registered Nurse License or a multi‑state Nursing License from a participating state in the Nurse Licensure Compact. American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA‑approved equivalent is required.
