Qualifications

• Requires clinical experience in mental health or human services.
• Regularly conducts assessments and develops treatment plans.
• Collaborates with multidisciplinary teams and coordinates care.

Responsibilities

• Provides case management services, including assessment, treatment planning, and ongoing support for clients with behavioral health needs
• Conducts diagnostic assessments and develops individualized treatment plans
• Coordinates care and collaborates with multidisciplinary teams, including providers, social workers, and nursing staff
• Delivers counseling, crisis intervention, and community-based support to clients and families
• Monitors client progress and adjusts treatment plans based on clinical findings
• Maintains accurate, timely clinical documentation in compliance with regulatory standards
