Job summary

Family Solutions is seeking a dedicated Mental Health Professional (MHP) to join our Chicago-based team. This full-time role focuses on person-centered, trauma-informed behavioral health services, delivered in a community setting with collaboration across care providers.

You will conduct comprehensive assessments, develop individualized treatment plans, provide outpatient therapy, crisis intervention, and case management while maintaining compliant documentation and high-quality care for clients

Qualifications

• Bachelor’s degree preferred; clinical care experience may also be considered.
• Experience in outpatient or community-based behavioral health setting preferred.
• Ability to provide assessment, treatment planning, therapy, crisis intervention, and case management.
• Certifications: First Aid, CPR, de-escalation, and required trainings.

Responsibilities

• Provide comprehensive clinical evaluations, including mental status and risk assessment.
• Support clinical assessment to determine service needs using DSM.
• Develop individualized treatment plans with clients, families and teams.
• Deliver outpatient therapy, crisis intervention, and case management services.
• Maintain timely, accurate, and compliant clinical documentation.
• Collaborate with care providers and community partners for better outcomes.
• Ensure confidentiality and compliance with regulations.
• Meet productivity standards with an average of 28 billable hours per week.
