Job summary

Diamond Experience Co. is seeking a Qualified Mental Health Professional/Case Manager to deliver Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families.

The QMHP will use a strengths-based model to help children and families function autonomously and successfully. The ideal candidate holds a CANS/ANSA certification, is bilingual in English and Spanish, and can navigate Medicaid guidelines, documentation, and EHR systems while

Qualifications

• Must have at least a Bachelor’s in Social Work, Mental Health Counseling, Psychology, Sociology, or related field.
• QMHP will provide Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families.
• CANS/ANSA Certification required or preferred (per job).
• Bilingual English/Spanish preferred.
• Must pass drug test and background check.
• At least two (2) years prior experience in behavioral health or closely related field.
• Must have reliable transportation, valid driver’s license, auto insurance and clean driving record.
• Access to computer and internet; ability to work independently with minimal oversight.
• Competent in MS Word, Excel, PowerPoint and EHR software.
• Ability to work from a strengths-based, trauma-informed, culturally competent perspective.

Responsibilities

• Adhere to Medicaid/Insurance guidelines.
• Collaborate with clinical team to develop service/treatment plans focused on child and family needs and strengths.
• Provide skill-building training in home or community settings using evidence-based practices.
• Offer crisis/emergency services via face-to-face, phone, or virtual contact.
• Maintain client documentation in the EHR and write progress notes per Texas Administrative Code.
• Enter data clearly, timely; complete daily session notes within 24 hours.
• Update client demographics in EHR by the 20th of each month.
• Assist in tracking outputs/outcomes; may help with record reviews.
• Use active listening to build rapport with clients and families; provide support.
• Keep family and team informed on progress toward Service Plan goals.
• Assist with daily living tasks to promote independent living.
• Collaborate with court/probation, tribal/community agencies, families, and supports.
• Participate in staff meetings and ongoing training.

The Qualified Mental Health Professional (QMHP) will provide goal directed interventions to stabilize children and youth in transition, and follow a strengths-based model that will help develop and build skills that will help the child and family function autonomously and successfully.

Responsibilities

• Must acknowledge and adhere to Medicaid/Insurance guidelines
• Work closely with the clinical team in developing a service/treatment plan that is focused on the child and family needs and recognizing their strengths
• Responsible for providing skill-building training in the home or out in the community using evidence-based practices and curriculums
• Provides support and crisis/emergency services for the child/family through face-to-face contact, phone or virtual
• Maintain all client documentation in the EHR and write progress notes adhering to requirements set by the Texas Administrative Code
• Enter data in a clear, accurate, and timely manner. Completion of daily session notes within 24 hours after session
• Documents non-billable contacts with the clients to show history of follow-up and/or non-complianceUpdates necessary client demographics information in EHR software by the 20th of each month
• Promptly submits documentation needed for the tracking of output/outcome measures. May be asked to assist in record review or qualitative review processes.
• Utilize effective listening skills to establish rapport with clients, family members, and other individuals close to the client; provide emotional support and encouragement; and engage family members in the service process
• Responsible for keeping the family and team members informed on progress toward achieving Service Plan goals
• Assist clients with daily living tasks such as behavior management, counseling services to the client, budget management, independent transportation assistance, time management skills, and other tasks to promote independent living
• Collaborate with court/probation, tribal/community agencies, families, and other natural support systems to ensure client’s needs are met. Develops a strong working relationship with community partners. Assist clients in practicing effective communication, problem-solving, and anger management techniques in their daily lives
• Participate in staff meetings, monthly training, company programs, professional development training as required
