Job summary

Urban Peak in Denver, CO is seeking a Housing Case Manager to provide supervised youth support, case management, and crisis counseling across shelter and housing settings. You will help youth develop service plans, coordinate with partner agencies, and document services in HMIS.

The role emphasizes trauma-informed care, safety, confidentiality, and collaboration with interdisciplinary teams. A master’s or relevant bachelor’s with experience in youth services is preferred; Spanish or other

Qualifications

• Master's degree in social work, psychology, or child development OR a bachelor's degree with relevant youth experience.
• CAC II certification preferred.
• Trauma-informed care and harm reduction knowledge required.
• Ability to maintain client confidentiality and accurate data.
• Experience with HMIS documentation and reporting.

Responsibilities

• Meets with assigned youth to obtain history and complete assessments.
• Provide trauma-informed guidance and support to youth.
• Develop individual service and aftercare plans with youth.
• Maintain accurate case notes and HMIS documentation.
• Collaborate with partner agencies for service delivery needs.
• Provide crisis intervention and on-site counseling as needed.
