Job summary

Covenant House Illinois (CHIL) seeks a Clinical Case Manager to deliver direct, trauma-informed services within Youth Development Center and Residential Programs. The CCM works with a multidisciplinary team to implement holistic service plans and supports youth toward stability and autonomy.

Reporting to the Director of Clinical and Supportive Services, the CCM collaborates with youth and partners to ensure quality, confidentiality, and compliance while promoting the agency’s mission and core

Qualifications

• Master’s degree in social work or related field required.
• 2-4 years of youth development experience required.
• Licensure in Social Work, Psychology, Counseling, or Human Services related field required.
• Experience with survivors of trauma, trafficking, or homelessness preferred.
• Strong trauma-informed care knowledge and resources awareness.
• Knowledge of community resources and housing programs.
• Excellent written and verbal communication; crisis judgment.
• Willingness to function as an active interdisciplinary team member.

Responsibilities

• Manage a caseload of youth in YDC and residential programs.
• Complete comprehensive assessments integrating trauma histories to develop holistic service plans.
• Collaborate with youth to develop short- and long-term ISP goals.
• Meet weekly with youth and document progress; convene Success Team meetings.
• Provide emotional support and strengths-based guidance to youth.
• Advocate for youth with outside service providers; coordinate appointments.
• Ensure youth have proper IDs and understand their rights; maintain confidentiality.
• Develop motivational tools and support youth in understanding resources and housing options.
• Facilitate groups on life-skills topics and track outcomes.
• Maintain strong relations with partners; assist with re-entry meetings after incidents.
