Job summary

Covenant House Georgia is seeking a Case Manager in Atlanta to provide ongoing support to youth (12-24) and families impacted by the criminal justice system. You will deliver 1-1 case management, life skills, group facilitation, and community engagement to build service plans for youth and family success.

The role supports crisis intervention, referrals, and collaboration with partners, with data entry in standard databases and adherence to program policies.

Qualifications

• BS in Human Services with 2-4 years’ experience working with youth
• Knowledge of mental health and substance use
• Valid driver’s license and ability to pass background checks

Responsibilities

• Develop individualized case plans with measurable goals and objectives based on needs identified in assessments
• Meet with youth, families, probation officers and court officials weekly and document interactions
• Participate with multi-disciplinary team to provide comprehensive service to youth
• Ensure integration of social services/case management functions for family support and referrals
• Organize and facilitate quarterly community engagement events
• Develop knowledge of community resources and attend meetings/training
• Maintain ethical and professional relationships with youth, families and partners
• Document timely in ETO, Client Track, APF Spreadsheet and Case Worthy per data integrity plans
• Provide crisis intervention and make informed decisions for youth infractions
• Promote efficient use of resources and assist in training staff
