Children's Home Society of South Dakota in Rapid City seeks a High-Risk Youth Case Manager to provide intensive case management, outreach, advocacy, and service coordination for youth and families affected by violence and trauma.

The role emphasizes trauma-informed, culturally responsive approaches to engage youth, support family stability, reduce risk factors, and connect to community resources, working within CVIPI to reduce violence exposure.

Responsibilities

• Build rapport and develop therapeutic relationships with youth and families to assess needs, stabilize crises, intervene appropriately, and advocate for services that address immediate concerns and promote long-term success.
• Provide trauma-informed case management, advocacy, and support services to youth and families impacted by violence, trauma, behavioral health concerns, truancy, justice system involvement, and other significant life challenges.
