Crittenton Center in Sioux City, IA is seeking an experienced Mobile Case Manager to provide intensive, trauma-informed case management for youth in various settings. You will build positive relationships and facilitate connections to school, housing, and employment opportunities while ensuring youth receive needed supports.

You will conduct intake and enrollment, plan aftercare, and maintain program fidelity with documentation and confidentiality.

Formación

• Bachelor’s degree in social work, psychology, or related field is required.
• Must pass criminal background check, abuse registry check, fingerprint check, and TB test.
• Valid driver's license and auto insurance are required.

Responsabilidades

• Create a caring, safe, and healing environment centered on the youth throughout their care.
• Complete intake/enrollment/assessment, aftercare planning, discharge forms and required paperwork.
• Coordinate enrollment location with referral staff/agencies and document tools.
