Job summary

Wisconsin Community Services seeks a Care Coordinator for the Wraparound program in Milwaukee. This role delivers intensive, individualized care coordination for children and adolescents, coordinating teams, plans of care, and crisis resources while maintaining strong community and family partnerships.

The position requires a bachelor’s degree in a relevant field and at least one year of mental health services experience, plus training, licensing, and strong communication skills.

Qualifications

• Bachelor’s degree in a relevant field and at least one year of mental health services experience.
• Complete 85 hours of mandatory Wraparound training within 6 months of hire.
• Windows and Microsoft Word proficiency.
• Demonstrated cultural competence with diverse populations.
• Valid driver’s license, automobile, and insurance meeting agency requirements.

Responsibilities

• Maintain a caseload of approximately 8 children and families.
• Coordinate initial visits within 5 business days of enrollment.
• Schedule and attend weekly visits and monthly Child and Family Team Meetings.
• Assemble a Child and Family Team within two weeks of enrollment.
• Develop initial Plan of Care within 30 days based on strengths and needs.
• Monitor service quality and liaise for new services/resources as needed.
• Advocate for children and families across home, school, court and community.
• Attend court hearings as a Wraparound Milwaukee representative.
