Job summary

Qualifications

• Bachelor’s degree in social work or related field required.
• 1 year experience with mental health, substance use, or co-occurring needs preferred.
• Valid driver's license and vehicle insurance per agency requirements.

Responsibilities

• Build trusting relationships with individuals, meeting them where they are.
• Use trauma-informed approach to coordinate supports.
• Develop and update individualized recovery plans with individuals and supports.
• Coordinate with service providers to ensure coordinated care.
• Monitor progress toward goals and provide coaching and support.
• Maintain regular contact through visits, calls, and follow-ups.
• Assist with transportation to program appointments as needed.
• Document all interactions accurately per standards.
